Chronic Disease Management
Patients who have a Chronic condition or disease such as diabetes, asthma, heart disease etc., can be assessed and monitored regularly to ensure they remain as healthy as possible and have the most up to date care and treatment.
Our Nurses assist with a range of comprehensive health assessments for people of all ages. This includes a thorough general health check up with both the nurse and doctor. Health Assessments or check ups are recommended routinely from the age of 40 years or earlier if indicated. Part of the Health Assessment generally includes a 'Cardiovascular & Diabetes Risk Assessment', to determine the risk of developing these conditions.
Jenny & Beth are accredited nurse PAP test providers. When you are next due for your PAP test you have the option of seeing either your regular GP or a nurse. Periodically we run after hours clinics for this service making it more convenient for those who are unable to attend during regular clinic hours.
The nurses work closely with the doctor to assist in providing optimal respiratory care, they have attended training for asthma management and lung function testing.
As fully qualified Nurse Immunisers our nurses are available to give all vaccines as per the Victorian National Immunisation Program to children up to 16 years of age. They are also available to give adult immunisation as required.
Our nurses continually update their skills and are qualified to assess, manage and treat wounds
